Do I need to fill out new patient forms before my first visit?
It helps to. You can download our new patient information form, complete it at home, and bring it with you — this saves time at check-in and lets Dr. Baldwin review your health history before your appointment. If you prefer, you can also arrive 15 minutes early and complete the paperwork in our Huntington Beach office.
What should I bring to my first dental appointment?
Please bring a photo ID, your dental insurance card (if you have coverage), a list of any medications you take, your completed new patient form, and any recent dental X-rays or records if you are transferring from another dentist. If you have questions about coverage, call us at (714) 536-2571 before your visit.
How early should I arrive for my first visit?
We recommend arriving about 15 minutes early. If you have already downloaded and completed the patient information form, check-in is quick and you will have time to settle in before meeting Dr. Baldwin and the team.
Do you accept my dental insurance?
We accept most PPO dental insurance plans and also offer CareCredit financing, HSA/FSA payments, and affordable cash-pay rates. Visit our Insurance & Payment Options page or call (714) 536-2571 and our team will verify your benefits before your appointment.

Dr. Baldwin’s office is at 8951 Atlanta Ave in Huntington Beach, CA 92646, inside the Vons Market Center with convenient parking. We are open Monday through Thursday 9 AM to 6 PM and Friday 9 AM to 1 PM.
